Registration

When registering, please bring your NHS medical card with you to the practice and those of any other persons registering with you. If you do not have a medical card you will be asked to complete a registration form (GPR) for each person registering with the practice. Essential document required when registering is proof of your address (recent utility bill or rent/mortgage agreement in your name). If you are newly resident in the UK or you are an overseas visitor then additional documentation is required, please contact reception for more information on this.  In addition, all patients will be asked to complete a new patient questionnaire which will allow us to provide medical care to you while your medical records are transferred from your previous practice. We DO NOT accept forms by post.

It is important for you to re-register with a new practice as soon as possible if you move out of our practice area.  Details of our practice area can be found at the bottom of this page.

Patients are registered with the practice and not with an individual GP.  For administrative reasons your medical card will be issued in the name of one of the doctors, however you can at any time express a preference for a particular doctor, for all of your medical needs or on a case by case basis.  This preference will be recorded in your medical records and we will do our best to respect your choice.  However, not all doctors in the practice provide all services and specific doctors may not be immediately available.
